Details: This academic paper requires students to write � up the patient presented at their Clinical Case Conference (CCC) to their Nurs3005 PEP facilitator and present it as a detailed report. There must be analysis of the patient�s medical condition(s) demonstrating the ability to apply theoretical concepts including (but not limited too) pharmacology; patho-physiology; anatomy and physiology. There must be presentation and evaluation of nursing and medical management of the patient. Sound clinical rationales must be provided that support the care afforded the patient. Appropriate evidence sources must be used and Harvard referencing used throughout. Students are encouraged to write up this report prior to undertaking their CCC presentations. Welcome to Lovanurse Memorial Hospital! You are a new hire RN with five years’ experience and have been assigned to a medical surgical floor that has high staff and patient turnover. In addition to standard medical-surgical patients, your floor also is assigned to do telemetry overflow if there are not enough beds available on the telemetry floor. Telemetry is full today.
As luck would have it, two of your RN colleagues have called in sick today. So, it’s you, the only RN, two LPNs, and one CNA. Your floor has 15 beds, which are full. Today, there are at least four possible discharges, six planned surgeries that will require admission, and a possible patient in the Emergency Department. So, this means that you’re going to have to work with the bed coordinator to shift patients to different units. Fun!
Your staff for today:
LPN 1: Macy: 40 year old Hispanic female who has been a LPN for 12 years. She speaks English but prefers to speak Spanish because her accent is rather strong. She volunteers to take Spanish speaking patients. She is a hard worker and handles delegation well. She does not know how to perform peritoneal dialysis procedures. She has not been IV certified.
LPN 2: Mike: 57 year old Caucasian male who is going through a divorce. He is known to resist delegation, especially from new staff. He is a smoker who takes at least 6 breaks a shift, regardless of permission from the manager or charge nurse. He is extremely smart and is thinking about going back to school to get his RN degree. He is a wiz at wound care and is IV certified, which means he can start IVs, give IV medications, etc.
CNA: Gilda: 24 year old Asian female who just finished her CNA training three months ago. She is quiet and because of her cultural beliefs often seems subservient. She is in school now to get her BSN but needed money to help her get through school. She is afraid to work with patients who are in a great deal of pain or male patients.
You are still acclimating to the hospital, charting/medication dispensing system, etc. Your manager is in Hawaii. The administrator on call is aware of what is going on and says that you have enough staff because you have 2 LPNs who can take patients.
It is 0700, and the off going shift nurses want to give report.
Your unit is set up in pods with 5 beds in each pod. Typically, each nurse takes a complete pod to help patients and other staff know who will be in charge of that area’s patients. However, it is okay to assign nurses to two pods, but not more than two. You must take a minimum of four patients. LPNs, at your hospital, may be assigned up to six patients as long as you review their assessment findings and notes. You must also cosign their notes at the end of the shift.

ok so .. include things about me being an ambitious individual who has admired and has been captivated by the role of a nurse. i grew up in a country that relied on the younger generations to look after their old family members so let that demonstrate how I’ve always been caring individuals. my reason for wanting to be a nurse is to gain skills, more experience and acquire vast knowledge that will help me treat and heal patients so they can return to living their lives independently. I studied an Access to higher education course with mature students and given their age,experience and perspective to current society meaning our opinions differed but i was still able to work well with them. my access course was in a different city and had to travel by bus and train that demonstrated dedication, commitment ,courage and
punctuality as i managed to finish and pass the course with good attendance. I’m currently studying a foundation degree to health and social care and i study anatomy and physiology,study skills, and work based learning which to my placement in a mental health inpatient ward. communication ,observing,documenting and handing over information to the nurse in charge is part of my role. i work with a multidisciplinary team there. I’m also currently employed in a private sector as a support worker for adults with learning difficulties and my duties involves personal care,assisting with meals,promoting independence and dignity,taking them to hospital appointments… can you link the information I’ve provided with the 6c’s of nursing. i think thats it for now

An appropriate Plan Design is the application of a performance improvement process.

This is the meat of the project where you will discuss the performance improvement process steps that you will employ to address the problem, taking care to include: Who, What, How, and Time Frame. Include in the discussion the process proposed to address the problem.

An example would be:

“The initial approach is to (1) collect accurate data on the extent of the problem, (2) brainstorm with staff, administrators and patients to determine to potential causes of the problem, and (3) conduct focus groups with current and separated staff to elicit additional contributing factors.”

The conclusion of the discussion should project the desired outcomes and future plans.
