Soap note 1 on Hypertension

 
In addition to Journal Entries, SOAP Note submissions are a way to reflect on your Practicum Experiences and connect these experiences to your classroom experience. SOAP Notes, such as the ones required in this course, are often used in clinical settings to document patient care. Please refer to the Seidel, et. al. book excerpt and the Gagan article located in this week’s Learning Resources for guidance on writing SOAP Notes.
After completing this week’s Practicum Experience, select a patient that you examined during the last 3 weeks. With this patient in mind, address the following in a SOAP Note:
Subjective: What details did the patient provide regarding his or her personal and medical history?
Objective: What observations did you make during the physical assessment?
Assessment: What were your differential diagnoses? Provide a minimum of three possible diagnoses. List them from highest priority to lowest priority. What was your primary diagnosis and why?
Plan: What was your plan for diagnostics and primary diagnosis? What was your plan for treatment and management including alternative therapies?
Reflection notes: What would you do differently in a similar patient evaluation?

Work individually to complete this assignment. You will be posting journal entries, one per book section, about your reaction to Where am I Wearing? The purposes of this journal are: to stimulate thought about the course material (even when you are not in class), to point out the relevance of the course concepts to everyday life, and to provide a starting point for class discussions about the material.

For entry, you should write a minimum of two paragraphs. The first paragraph should provide a brief overview of the chapters in that section (e.g., what happened in the chapter, what countries/factories he visited, which people he interacted with, etc.). The second paragraph should provide your reaction to the chapter. Exactly what you write in the second paragraph is up to you. Some things that you might consider writing about include, but are not limited to: how the material covered in the chapter relates to the material covered in the class, how the material covered in the chapter changed or strengthened your ideas or opinions about global apparel sourcing, how the material covered in the chapter relates to an experience you have personally had, how the material covered in the chapter will influence your future decisions as an apparel professional, or any questions that you have about the chapter or the course materials.

The focus in health care reform is cost control in light of annual double-digit inflation since the late 1990s and the consumption of nearly 17% of the gross domestic product (GDP) in 2009 (more than any other country in the world). The debate is over how to reduce the rate of spending for health care while preserving quality and access to care for patients. Research at least 2 methods of reducing the percentage of the GDP spent on health care. You will need to be able to describe how you would do this in specific terms. This can be a number of approaches, such as the following:

Completely dismantle the current system and start over—A monumental task
Revise the current payment system to reflect current economic constraints
Cut Medicare and Medicaid
Insurance reform
National health insurance based on the Massachusetts Model
Malpractice reform
Legislation such as the Balanced Budget Act of 1997, the Medicare Modernization Act of 2003, the Affordable Care Act (2010), and others

A Belgian chocolate maker, part of bigger multinational food company, is considering purchasing a small vessel to import cocoa from western Africa.
A dry bulk ship will have to make the return trip carrying ballast; a general cargo could carry some of its parent company’s products (but not dairy products, which means it might have to undertake return trips below maximum capacity); a breakbulk reefer would guarantee maximum utilisation of the ship’s capacity.
You are asked to consider the yearly costs (Bunkering, Operational, Insurance, Port etc.) for 3 handysize vessels (dry, general, reefer) operating on the Abidjan-Antwerp route. Most of these costs can be quantified using Drewry’s Ship Operating Costs Annual Review and Forecast, available in the library.
Your report is expected to include an appendix table summarizing the associated costs in a single page for easy comparison, as well as a recommendation based on said cost comparison.
