The area of evidence based practice of my interest is pain management in sickle cell disease patients. Sickle cell disease arises as a result of abnormal red cell mutation. According to evidence-based research, people who inherit two Hb S genes from their parents are more than likely to have sickle cell disease. Deoxygenated Hb S tends to deform the erythrocyte, morphing into an abnormal red cell shape known as sickle cell. Due to the abnormal shape of hemoglobin in sickle cell patients, circulation is often impaired. This blockade of blood flow normally leads to tissue ischemia and other co-morbidities manifesting in Sickle cell patients.

One of the major problems of sickle cell patients is pain which is mostly as a result of blood vessels being occluded. For most patients, the pain is often sporadic and it varies in frequency and severity. The pattern of pain, intensity and severity tends to vary over time for most patients which make management of pain a little complicated. Another major problem is the lack of objective pain index. In addition to the non-stable characteristic of pain, the mode of onsets and sites also varies among patients. For some, development of severe pain can occur within 10 minutes and resolve within a few hours; while in others, it can escalate over days. In addition to the above stated problems, self-medication and non-compliance also factors into why pain management in sickle cell patients is complicated.

Due to the fact that there is no objective pain index that can be used by care providers to assess patient’s pain, providers trust in the patient report as key to the management of sickle cell pain crises. Furthermore, Patient education on sickle cell disease and crises prevention is crucial to the effective management of pain in sickle cell. Patients should be encouraged to be active participant in their care and management of their disease process. In order to effectively manage pain in sickle cell patients, medication compliance and other management therapy should be well monitored by care provider.
