Telenursing Nursing Informatics

SCENARIO – The following scenario serves as the basis for course work paper: You have worked with Tomika for the past 5 years. Tomika shares with you that she has resigned and plans to work in an agency that installs telemonitoring equipment into the homes of those with chronic illnesses. Nurses monitor the patients using the equipment with the goal of detecting problems before patients need to be readmitted to the hospital. Tomika will be working from her own home, with occasional meetings at the agency. She would not be visiting her patients in their homes, but rather would be assessing and interacting with them via videoconferencing. She tells you that there are still job openings and encourages you to apply. You are intrigued by this, and decide to investigate whether telenursing would be a good choice for you, too. Is telenursing in your future?
DIRECTIONS:
1. You are to research (find evidence), compose, and type a scholarly paper that describes telenursing as described above, and whether it is a good fit for you. Reflect on what you have learned in this class to date about technology, privacy rights, ethical issues, interoperability, patient satisfaction, consumer education, and other topics. Your text by Hebda (2013, Chapter 25) discusses telehealth in detail. However, your focus should be from the professional nurse’s role in telenursing. Do not limit your review of the literature to only what you read in your text. Nurses in various specialties need to know about the advantages and disadvantages of telenursing as it applies to their patients. For example, when you discharge a patient from an acute care setting, will a telenursing service assist that individual with staying out of the hospital? You may need to apply critical-thinking skills to development of your paper. In the conclusion of your paper, describe your current employment situation, and whether a job in telenursing would, or would not, fit with your career goals and life situation once you graduate from Chamberlain.
2. Use Microsoft Word and APA formatting to develop your paper. Consult the Publication manual of the APA, 6th edition if you have questions, for example, margin size, font type and size (point), use of third person, and so forth.
3. The paper should be four to five pages, excluding the title page and the reference page. Limit your references to key sources. Paper should contain an introduction that catches the attention of the reader with interesting facts and supporting sources of evidence, which need to be mentioned as in-text citations; i.e., According to Hebda (2013, p. 9), she states the following regarding the use of electronic resources…
4. Keep in mind that APA guidelines state you are not to call this an Introduction, but you should include it at the beginning of your paper. The Body should present the advantages and disadvantages of telenursing from your perspective as an employee, and the patient’s perspective as a recipient of the care nurses provide. The Conclusion and Recommendations should summarize your findings and state your position on whether you will apply for a position with the agency.
5. You are to research/find evidence, compose, and type a scholarly paper that describes telenursing as described above, and whether it is a good fit for you. Reflect on what you have learned to date about technology, privacy rights, ethical issues, interoperability, patient satisfaction, consumer education, and other topics. However, your focus should be from the professional nurse’s role in telenursing. Do not limit your review of the literature to only what you read in your text, but should quote Hebda at least once in your paper. Nurses in various specialties need to know about the advantages and disadvantages of telenursing as it applies to their patients. For example, when you discharge a patient from an acute care setting, will a telenursing service assist that individual with staying out of the hospital? You may need to apply critical-thinking skills to development of your paper.

WEEK 4 This is a overview for this week .

Study Notes

CHAPTER 5: BIOLOGICAL ASPECTS
This chapter is unique in providing a comprehensive overview of biological influences on personality, but it is written in a way that is tied to other approaches and is easily understandable by students. For example, this chapter does not get into esoteric disputes from biology and genetics, but rather keeps the focus on personality. It can easily be integrated into courses that have not previously included a chapter on this topic. With the ‘unravelling’ of the human genome in the year 2000, and the development of the field of behavioral genomics, it is increasingly important for students to understand biological aspects of personality.

Evolutionary effects and genetic abnormalities are of course important, but many biological influences are not genetic! For example, environmental toxins, physical disease, and biologically-based creations of environments and expectations are important biological influences on personality. This is too often ignored. This chapter challenges students to think for themselves about the role of biology, and re-evaluate common stereotypes. Importantly this chapter concludes with warnings about the political dangers of a simple-minded approach to biology and personality.

??

Possible Lecture Outline

I. Direct genetic effects
A. Through process of natural selection certain adaptive or functional characteristics have been reinforced
1. These characteristics include behavioral tendencies and emotions— “personality”
B. Example of Angelman syndrome as a “personality” directly caused by genetic abnormality
C. Behavioral genomics is a new area of study that examines relations between genes and behaviors
II, Effects through temperament (activity, emotionality, sociability, aggression/impulsivity)
A. Eysenck’s theory links the introversion-extroversion dimension to the underlying nervous system
1. posits introverts have a higher level of internal arousal and thus seek out less external stimulation (and vice versa)
B, Jeffrey Gray and his colleagues suggest that two biological systems, the behavioral inhibition system and the biological activation systeminfluence our preferences for different types of stimulation
1. the behavioral inhibition system guides our tendency to avoid potentially punishing situations while our biological activation system regulates our response to rewards
C. Zuckerman’s theory posits that those high on “sensation seeking” are driven by a low level of internal arousal; sensation seekers are drawn to novel and exciting experiences
1. sensation seekers may have a low level of natural (internal biological)
activation and so seek arousal from the environment
2. it is likely that some people have natural defects or disease-caused weaknesses in their dopamine systems, and such people may even be susceptible to addiction
3. the neurotransmitter serotonin also seems i-elated to impulsivity
U Studies of twins reared in different environments have demonstrated impressive similarities between those with the same genetic make-up
1. Still unclear how much of the similarity is genetically pre-programmed and how much is due to similarities in their separate environments, etc.
2. Twins and siblings do not necessarily experience the same rearing environments
a. Nonshared environmental variance are those features of the environment that children experience differently
3. Twin studies have helped to show that schizophrenia is genetically linked, but it’s clear that it’s not simply a genetic disease
a. structural abnormalities have been found in the brains of schizophrenics
b. concordance between identical twins is far from perfect
4. Twin studies have also shown that homosexuality is linked to genetics a. again genes do not tell the whole story
b. structural differences have also been found between the brains of homosexual as compared to heterosexual men
c. how might homosexuality be addressed from an evolutionary point of view? Perhaps through “kin selection” or some other yet-to-be-discovered process

Ill. Effects through environmental toxins and physical illness
A. Toxins/drugs
1. “Mad as a hatter” derived from brain damage hatters suffered when
exposed to mercury in hat-making factories
2. Lead poisoning and cognitive/behavioral deficits in children
3. Manganese miners and fighting behavior
4. Personality changes associated with illegal or prescription drugs
B. Disease
1. Van Gogh and the possibility of Meniere’s disease
2. Personality changes as a result of Alzheimer’s disease
3. Picks disease is an example of dramatic change in a patient’s sense of self, long before total incapacity
Personality changes following stroke or CABG

Consider the clinical environment in which you are currently working or have recently worked. Collaborate with a leader or educator in the clinical environment to identify a problem, issue, or educational deficit upon which to build a proposal for change.

In a paper of no more than 800 words, describe the nature of the problem, issue, or educational deficit. The problem I’d like to address is Bedside Nursing Report. I’m currently an RN working on a med/surg floor and many nurses only give report at the nurses station. Include the following in your discussion:

The setting and/or context in which the problem, issue, or educational deficit can be observed.
Detailed description of the problem, issue, or educational deficit.
Impact of the problem, issue, or educational deficit on the work environment, the quality of care provided by staff, and patient outcomes.
Gravity of the problem, issue, or educational deficit and its significance to nursing.
Proposed solution to address the problem, issue, or educational deficit.

Pain Management

Case Study: Mr Jones
Frank Jones is a 48 year old man presenting with low back and left leg pain.
History
Mr Jone was working as a motor mechanic when lifting a heavy piece of machinery
weighing 35 kg (77 lb) on 5 March 2002. He felt something “give” and had
immediate onset of pain in the low back. He was seen by his local doctor and given
antinflammatory medications and told to take 3 days off work. There was no
neurological deficit and X rays of his lumbar spine failed to find any abnormality.
He requested several more days off work and then returned to work one week after
the injury. Any lifting caused an increase in his pain and after consultation with his
local doctor he was placed on light duties. He returned to work and was given office
duties but his boss made it clear that he was not welcome at work if he could not
return to what he was doing before. He was very dissatisfied doing paperwork.
Eight weeks later his pain had largely resolved and he was cleared by his local
doctor and allowed to return to normal duties. Three months later when he bent over
to pick up his tool box he experienced pain in his back and a shooting pain down his
left leg.
He returned to the local doctor who requested a CT scan. This showed a left
posterolateral L4/5 disc protrusion that was impinging on the left L5 nerve root.
He was refered to an orthopaedic surgeon who advised an L4/5 discectomy.
Following surgery, he had good relief of the leg pain although not the back pain and
despite attempts to return to work found it impossible to carry on. He was dismissed
from his job at the mechanical workshop six months later.
He now spends much of his time at home and twelve months following surgery
began to experience constant burning pain in the left leg below the knee. He now
has continuing pain in the low back and left leg.
